Reinforcement steel fixing involves the placement, tying and securing of deformed reinforcing bar and mesh to form the structural cage for concrete elements β footings, pile caps, slabs, walls, columns, beams and suspended decks. Steel fixers work with sharp bar ends, heavy bundles slung from cranes, tying wire under tension, and increasingly with rebar tying tools and shear-fed cutting equipment. The work exposes workers to impalement on protruding starter bars, crush injuries during craned lifts, musculoskeletal disorders from repetitive stooping and tying, lacerations, and falls from height when fixing to elevated slabs, wall cages or bridge decks.
Under the Work Health and Safety Regulation 2025 (NSW), reinforcement fixing on multi-storey slabs, suspended decks and deep footings is high risk construction work under section 291(a) where a person could fall more than 2 metres. Section 299 requires a Safe Work Method Statement to be prepared before the work commences, kept available for inspection, and complied with. Controls must follow the hierarchy under section 36, with elimination of fall exposure and impalement risk prioritised over administrative measures and PPE.

Control measures

- 1Eliminate fall exposure where reasonably practicable by prefabricating reinforcement cages at ground level and craning the completed cage into position rather than tying at height.
- 2Where fall risk cannot be eliminated, install perimeter edge protection compliant with AS/NZS 4994.1 to all slab edges, penetrations and lift shafts before steel fixing begins; use fall-arrest harness systems compliant with AS/NZS 1891.1β1891.4 anchored to engineered points only where edge protection is not reasonably practicable.
- 3Fit approved impalement protection (mushroom caps or troughed timber capping) to every projecting vertical starter bar until concrete is poured or bars are cut off; caps must be rated to resist a falling worker, not decorative plastic sleeves.
- 4Engineer wall and column cages with temporary bracing, kickers or tie-back struts designed by a competent person to prevent collapse before concrete placement; cages over 1.5 m tall must not be left freestanding without bracing.
- 5Plan and separate craned lifts of bar bundles and prefabricated cages under a lift study; use certified lifting gear inspected to AS 4991, tag lines, and exclusion zones with no workers under the load. Doggers and riggers must hold the relevant high risk work licence.
- 6Confirm location of underground services by DBYD search and hand-dig proving before driving starter bars; maintain no-go clearances from overhead powerlines in accordance with the SafeWork NSW Code of Practice: Work near overhead and underground electric lines and the network operator's requirements.
- 7Reduce manual handling injury by mechanising delivery to the work face (materials hoist, kibble, telehandler), limiting bundle weight, rotating tying tasks, and issuing battery-powered rebar tying tools to reduce wrist load.
- 8Maintain housekeeping: bundle and remove tie-wire offcuts, provide walkway boards over congested top mats, and ensure all bar tails and mesh ends are turned down or capped.
- 9Implement heat stress controls per the SafeWork NSW Managing the risks of working in heat guidance: shaded rest areas, hydration, work-rest cycles and rescheduling of high-exertion tasks during forecast extreme heat.
- 10Issue and enforce task-appropriate PPE: cut-resistant gloves (EN 388), safety eyewear to AS/NZS 1337.1, safety footwear to AS/NZS 2210.3 with penetration-resistant midsole, high-visibility clothing to AS/NZS 4602.1, and long sleeves plus SPF 50+ sunscreen for UV.
- 11Provide a documented pre-start inspection of edge protection, void covers, bar caps, cage bracing and lifting gear each shift; suspend work on any identified defect until rectified.
- 12All workers to hold General Construction Induction (CPCCWHS1001 White Card) and be inducted to this SWMS. Consult workers under section 47 of the WHS Act during preparation, and review the SWMS under section 302 of the WHS Regulation whenever the work method changes, controls fail, or a notifiable incident occurs.

High-Risk Construction Work triggered
Reinforcement fixing routinely occurs on suspended slabs, elevated decks, wall cages, bridge soffits and at the edge of deep footing excavations where the fall distance from the working surface exceeds 2 metres, engaging section 291(a) of the WHS Regulation 2025 (NSW).
Because the work is HRCW under section 291(a), a SWMS complying with section 299 must be prepared before work starts, complied with during the work, and reviewed under section 302 if controls are revised or an incident occurs. Failure to prepare, comply with or review a SWMS is an offence attracting penalties under the WHS Regulation. Where the failure exposes a worker to a risk of death or serious injury, prosecution may proceed under sections 31 (Category 1 β reckless conduct) or 32 (Category 2) of the WHS Act 2011 (NSW). Current maximum penalties are published in the SafeWork NSW penalty notice schedule and are periodically indexed.

Worked example
A steel fixing subcontractor is engaged on a six-level residential build in Rhodes, NSW. The team is fixing the top mat of the Level 3 suspended slab, with a 4.2 m fall to the Level 2 slab below at unprotected penetrations and lift core. Prior to commencement, the subcontractor's supervisor consults the crew under section 47, walks the deck to confirm edge protection to AS/NZS 4994.1 is installed, verifies all vertical starter bars from the Level 2 columns are fitted with troughed timber capping, and confirms void covers are fixed and signed. Prefabricated column cages for Level 4 are tied at ground level in a laydown area and craned into position by a licensed dogger with an exclusion zone barricaded. Battery-powered rebar tying tools are issued to reduce wrist strain. When a subsequent design change requires additional trimmer bars around a new penetration, the SWMS is reviewed under section 302, the change consulted with workers, and the amended version signed before the varied work proceeds.

Frequently asked questions
Is reinforcement steel fixing always high risk construction work?
Not automatically. It becomes HRCW when a section 291 trigger applies β most commonly section 291(a) risk of a fall more than 2 m on suspended slabs, decks or deep footings, or section 291(g) where work is adjacent to a road or trafficked area, or section 291(k) where the excavation depth exceeds 1.5 m. Ground-level slab-on-grade fixing with no adjacent excavation may not trigger HRCW, but a SWMS is still best practice and the general risk management duty under section 34 still applies.
Are plastic mushroom caps sufficient impalement protection?
Standard decorative plastic caps are not adequate on their own to prevent impalement from a falling worker. Where a fall onto starter bars is foreseeable, use troughed timber capping or engineered impalement-rated caps that spread the load, or eliminate the exposure with fall prevention. The SafeWork NSW falls Code of Practice and current guidance make clear that light plastic caps are visual markers, not fall-protection devices.
Who must sign the SWMS?
Every worker performing the reinforcement fixing task must sign on to the SWMS before starting work, confirming they have been consulted on and understand the controls. The person conducting the business or undertaking (PCBU) preparing the SWMS is responsible for consulting workers under section 47 of the WHS Act and ensuring the document is available at the workplace under section 300 of the WHS Regulation 2025.
When must the SWMS be reviewed?
Section 302 of the WHS Regulation 2025 (NSW) requires review whenever the control measures are revised, the work method changes, a new hazard is identified, a notifiable incident occurs, or a health and safety representative reasonably requests it. On a typical project this means reviewing before each new floor pour, when new plant such as a mobile crane is introduced, or when the design is varied.
