Underground service strikesFatalDBYD enquiry, service plans reviewed, potholing to expose, hand-dig within tolerance zone, reduced bucket speed near servicesAdministrative / Engineering
Rollover / tip-overFatalOperate on stable ground, track positioning on slopes, outriggers where fitted, no operation on >15° slope without assessmentEngineering / Administrative
Falling objects from bucketSeriousNo workers in swing radius, loads secured in bucket, bucket lowered during pauses, check lug and pin condition dailyAdministrative / Engineering
Hydraulic failure / hose burstSeriousDaily pre-start inspection (hose condition, hydraulic levels, pin/bush wear), no body parts under raised implements, operator to stay in cab if failureEngineering / Administrative

Frequently Asked Questions

Q:Does an excavator operator need a licence?
Excavators over 3 tonnes do not require a high-risk work licence, but operators must be trained and assessed as competent. Excavators used for crane duties (lifting and slewing) may require a crane licence depending on the rated capacity.
Q:What is the exclusion zone for an excavator?
The exclusion zone should be the full slew radius of the excavator plus a minimum 3-metre buffer. No workers should enter the exclusion zone while the excavator is operating unless in direct communication with the operator.
